Help with recruitment survey

I am in charge of recruitment for Panhellenic and I am working on a survey to email to each chapter (that the presidents will in turn forward to their girls) that will give each girl an opportunity to express her thoughts about recruitment and Greek life. Although, I have already discussed many thoughts/ideas in person with some of the actives in each chapter, I'd like to give each girl a chance to express herself and feel like she's heard. Of course, a survey is much more informal than in person, so I am trying to brainstorm questions I could ask the girls. Specifically, I'd like to hear any ideas related to things they would like me to change or do differently for recruitment / Greek life. Any thoughts on questions I can ask in this survey? Thank you for any thoughts/help!

Maybe: Do you know any women that would be a great asset to any of our chapters but chose not to participate in formal/informal recruitment? If so, what were some of their determining factors?
